Failure to turn in this form may result in the account being disabled and removed.
Our policy is that
departed deptartment members have their accounts closed after 3 months
of leaving.
If you need an
exception, email a formal request which explains the purpose of a further
extension to chair@physics.syr.edu
and sysadmin@physics.syr.edu.

email can still be forwarded with the account being deactivated. Please
don't request the account to be left open if you are no longer associated
with SU Physics, although you can request a short time to be able to get
any files needed before they are cleared.

The Department furnishes e-mail addresses to most faculty, staff (including
research associates), and graduate students. When members of the Dept.
leave, the Dept. will continue to provide e-mail service for at least 3
months. After this period, an address to which e-mail may be forwarded
needs to be supplied. The Dept. will continue to furnish this forwarding
service indefinitely; the purpose is to support e-mail related to published
work, which can lead to e-mail correspondence long after a dept. member
has departed.
